WTOP radio, 7/27:

Another Metro train may have run red signal

WASHINGTON — A Metro train may have overrun a red signal Wednesday afternoon, in what would be just the latest lapse in the system.

Metro said in a message provided to WTOP by a source that a train traveling outbound from the Reagan National Airport station overran a red signal, and rail traffic was then stopped in the area.

WTOP radio, 8/3:

Metro fires another train operator for running red light

WASHINGTON — A train operator who drove a train past a red signal at Reagan National Airport last week has been fired, after Metro’s internal investigation found the cause of the incident was “operator error.”

And another one???

From Greater Greater Washington, 8/17/16:

Train operators continue running red signals on Metro, say the feds

Metro trains have run red lights 68 times in the last four and a half years, according to the Federal Transit Administration (FTA). The number of incidents per year isn't going down, and while different types of human error have been the immediate causes, poor training and culture are underlying factors.

The Washington Post, 10/24:

Metro’s answer to train operators running red signals: brighter bulbs

Metro has issued a series of recommendations aimed at fixing its red light-running problem, including capping train speeds and installing brighter bulbs at the site of frequent violations, according to an internal review released by the transit agency Monday.

The report, an overview of 88 red-signal overruns dating back to 2011, sheds light on the times, places and conditions under which Metro trains were most likely to run red lights. It comes two months after the Federal Transit Administration issued an urgent safety directive with 11 corrective actions the agency must take to prevent signal overruns.
