Amtrak responds to FRA safety directive following deadly accident


The Federal Railroad Administration (FRA) has directed Amtrak to conduct an immediate safety review with key workers, including track workers and train dispatchers, as the regulator continues its investigation into this week's fatal train accident in Chester, Pa.

Amtrak President and Chief Executive Officer Joseph Boardman said yesterday in a prepared statement that the railroad agrees with the FRA directive and is moving to take immediate action.

Unions blame corporate changes at Amtrak for worker deaths

 

PHILADELPHIA (AP) -- Railroad union leaders say a "perfect storm" of corporate changes at Amtrak is largely to blame for two workers being killed on tracks near Philadelphia.

 

In a letter sent Wednesday to Amtrak President Joe Boardman, maintenance-of-way and other union officials say employee training is inadequate and demand a change in the policy on "close call" reporting of dangerous incidents.

 

They say hazardous conditions exist on the main Northeast Corridor.

The information in this report is preliminary and will be supplemented or corrected during the course of the investigation

 

On Sunday, April 3, 2016, at 7:50 a.m., National Railroad Passenger Corporation (Amtrak) passenger train 89 struck a roadway maintenance machine (backhoe) at milepost 15.7 on the Amtrak Northeast Corridor near Chester, Pennsylvania.[1] The train consisted of one locomotive, eight passenger cars, one café car, and one baggage car. Amtrak train 89was operating on main track 3 at 106 mph, within the maximum authorized speed of 110 mph. The locomotive struck the backhoe, and the debris struck and damaged the passenger cars. The backhoe operator and a supervisor died. The train had 7 crewmembers and 337 passengers on board at the time of the accident, including two employee passengers; 41peoplewere transported to local hospitals.

 

The weather at the time of the accident was scattered clouds, winds from the west at 38mph gusting up to 50 mph, and a temperature of 37°F. Amtrak estimated damages of $2.2million.

 

The locomotive engineer stated to National Transportation Safety Board (NTSB) investigators that immediately prior to the accident, he initiated an emergency brake application after “seeing something” on main track 3.

 

At the time of the accident, roadway workers were performing maintenance on main track 2 at the accident site over a planned 55-hour window, beginning on April 1, 2016, at 10:00 p.m. and extending to 5:00 a.m. on April 4, 2016. The work included ballast cleaning and remediating fouled ballast (mud spots) on main track 2.[2] During the maintenance window, main track 2 was removed from service and intermittent foul time was granted on main tracks 1, 3, and 4 to protect the backhoe as it was used to assist in cutting away the fouled ballast.[3] NTSB investigators are confirming what roadway worker protections were in place at the time of the accident.

Parties to the investigation include the Federal Railroad Administration, Amtrak, Brotherhood of Locomotive Engineers and Trainmen, International Association of Sheet Metal, Air, Rail and Transportation Workers, the Brotherhood of Maintenance of Way Employes Division, and the American Railway and Airway Supervisors Association.

 

1 In this report, all times are eastern daylight time.

2 Ballast degrades over time and can cause poor drainage and mud to appear in the track structure, compromising track surface and stability.

3 Foul time is a method of establishing working limits on controlled track in which a roadway worker is notified by the train dispatcher that no trains will operate within a specific segment of track until the roadway worker reports clear of the track.


Probable Cause

Report: Track was closed morning of Amtrak crash

Wilmington worker was killed in April 3 crash south of Philadelphia

 

A backhoe and two longtime Amtrak employees, one from Delaware, were authorized to be on the railroad tracks on April 3 when a passenger train hit and killed them, federal investigators said Monday.

 

In a preliminary report, the National Transportation Safety Board said the workers were performing maintenance on track 2 in Chester, Pennsylvania. The work was scheduled to take place from 10 p.m. April 1 until 5 a.m. April 4. At the time of the crash, the track was supposed to be closed to trains, the report said.

 

"NTSB investigators are confirming what roadway worker protections were in place at the time of the accident," the agency said.  .  .  .

Family of worker killed in Delaware County Amtrak crash files suit

 
The family of an Amtrak employee killed when a train hit a backhoe he was operating in Delaware County filed suit against the national rail company Thursday, blaming negligence for his death.

NBC10 Investigators: Safety Device Missing from Amtrak Site After Deadly Accident

 

An NBC 10 Investigation has revealed a basic safety device called a shunt missing at the site of Amtrak’s most recent deadly accident on the Northeast corridor.

“Unfortunately, you’re always going to have some kind of human error and nobody’s life should be one step away,” Jedd Dodd, the General Chairman of the union representing Amtrak maintenance workers said.
 
Dodd, who worked on Amtrak maintenance crews for seven years, said a simple electrical cable put on the track called a shunt would have turned the oncoming railroad traffic signal red – telling trains to stop.

Amtrak engineer in Pa. train crash tested positive for marijuana, opioids

 

 

WASHINGTON, D.C. — The engineer of Amtrak train 89, which derailed in Chester City, Pa. in April, killing two, tested positive for marijuana and opioids, The Inquirer reports.

 

The findings are part of the information released on Thursday by the National Transportation Safety Board, with the agency expected to release their conclusions on the cause of derailment later this year.
