SEPTA did have one or two horrific escalator accidents a few years ago, of which I believe at least one was fatal. The Philadelphians in our group can help me out with that fact.

A rare occurrence that an escalator was actually working (although lately the ones in Market East and 30th Street Station have been pretty reliable when I've been there).

I can vaguely remember one such accident. In the mid-90's, a boy had his foot severed by an escalator on the Broad Street Line in North Philly (I don't remember which station specifically). A court ultimately forced SEPTA to pay the boy's family millions of dollars in damages.
