How could the train keep going if part of the wheels or breaks malfunctioned in that manner? I'm surprised there was no derailment or emergency break application.

In the most benign scenario, it appears that someone forgot to inspect the integrity of the assembly of tread brakes. Those flying away are unlikely to cause a huge problem. OTOH, if this was a disk brake falling apart that is a different kettle of fish altogether.
